8) PAYMENTS AND FINANCIAL INFORMATION (INSURANCE AND CASH PAY)
8.1 Insurance billing
If you use insurance, we bill your insurance and may collect and use information necessary to verify benefits, submit claims, and support payment and administrative processes.
8.2 Cash pay (in-person Square terminal)
If you do not have insurance, we accept cash-pay. Cash-pay is collected in person at our clinic prior to services being rendered. In-person card payments are processed through a Square terminal. We do not intentionally collect full payment card numbers through the Site for in-person Square payments.
Square’s processing and protection of payment information is governed by Square’s own policies and security practices.
9) SMS/TEXT AND EMAIL COMMUNICATIONS
9.1 SMS/text messages
If you opt in to receive SMS messages:
- We may send service-related messages such as scheduling, reminders, and next steps.
- Message and data rates may apply.
- You can opt out by replying STOP (or using any opt-out method described in the message).
