The right support can change
as your needs change.

Continuum of care means matching the level and type of support to what is happening now, while helping care remain connected when needs, goals, or circumstances change.

One Connected Approach

Continuity is not about staying in one program.

It means receiving the level of support that may be appropriate now, understanding when needs should be reviewed, and helping important information and next steps remain connected when care changes.

  1. The Right Intensity

Support may become more or less intensive depending on symptoms, safety, daily functioning, progress, and clinical recommendations.

  2. Connected Transitions

When a different service or setting is discussed, clear communication can help reduce confusion and support a more organized transition.

  3. Ongoing Review

Care plans may be reviewed as new information becomes available or a person’s needs, goals, response to care, or circumstances change.

Levels of Support

Different levels of care serve different needs.

The continuum includes settings with different levels of structure, frequency, monitoring, and independence. The appropriate level depends on the individual situation.

  1. 01

    Immediate or Emergency Support

For situations involving immediate medical or safety concerns.

  2. 02

    Inpatient or Residential Care

A highly structured setting that may be discussed when around-the-clock support, stabilization, or a higher level of supervision is appropriate.

  3. 03

    Partial Hospitalization or Day Treatment

Structured treatment occurring for much of the day while the person continues living outside the facility.

  4. 04

    Intensive Outpatient Care

Structured outpatient treatment that may include multiple treatment contacts while allowing the person to continue living at home.

  5. 05

    Standard Outpatient Care

Scheduled appointments that may include psychiatric care, counseling, medication support, TMS, or other outpatient services based on the individual plan.

This spectrum is educational and is not a placement recommendation. The appropriate level of care depends on individual needs, clinical evaluation, safety, eligibility, service availability, and other requirements.

Stepping Up, Stepping Down, or Changing Direction

A change in care is a response to need— not a judgment.

Recommendations may change as symptoms, safety, stability, functioning, progress, barriers, and circumstances become clearer.

When More Support May Be Discussed

  • Increasing safety concerns
  • Symptoms becoming harder to manage
  • Difficulty functioning in daily life
  • Current services no longer providing enough support
  • A need for closer medical or psychiatric monitoring
  • Significant changes in substance use, stability, housing, or support
  • A recommendation following a new assessment or review

When Less Intensive Support May Be Discussed

  • Greater stability
  • Improved ability to manage symptoms or routines
  • Progress toward treatment goals
  • Reduced need for frequent appointments
  • Stronger use of coping skills and community support
  • Readiness for greater independence
  • A care-team recommendation based on current needs
  1. 01

    Review What Is Happening

The care team gathers updated information about needs, safety, progress, barriers, and current functioning.

  2. 02

    Discuss the Recommendation

The person receives an explanation of why a different service, schedule, or level of care may be considered.

  3. 03

    Coordinate the Next Step

With appropriate permission, the team may help with records, referrals, insurance-related requirements, appointments, and communication with another provider.

Common questions

Continuum of Care FAQ

Learn how levels of care may be reviewed, how transitions may be coordinated, and where AriseVida Care’s outpatient services fit within the broader continuum.

  • No. The appropriate starting point depends on current needs, safety, clinical recommendations, service availability, and other requirements. Some people begin with standard outpatient appointments, while others may first require a more structured setting.

  • Yes. The level, frequency, or type of support may be reviewed as symptoms, stability, progress, barriers, safety, or other circumstances change. Any adjustment should be discussed with the appropriate care-team member.

  • No. AriseVida provides outpatient services. When a service or level of care is not available through AriseVida, the team may explain the recommendation and, when appropriate, help identify an outside provider or another possible resource. Availability, admission, insurance participation, placement, and a specific start date cannot be guaranteed.

  • Coverage varies by plan, service, provider, medical-necessity criteria, authorization requirements, and other payer rules. Verification can help explain available information, but it is not a guarantee of coverage, approval, or payment.

  • Family members, trusted supporters, hospitals, or other providers may sometimes participate when appropriate and when required consent or authorization is available. Privacy and confidentiality requirements still apply.

  • Follow-up may include returning to outpatient treatment, medication support, therapy, case management, peer support, or another recommended service. The exact plan depends on the person’s needs and available services.
