You have the right to receive a Good Faith Estimate

Under federal law, health care providers must give patients who do not have certain types of health care coverage, or who choose not to use their coverage, an estimate of their bill for health care items and services before those items or services are provided.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any health care items or services upon request or when scheduling such items or services. This includes related costs like medical tests, prescription drugs, equipment and facility fees.
  • If you schedule a health care item or service at least 3 business days in advance, we give you a Good Faith Estimate in writing within 1 business day after scheduling. If you schedule at least 10 business days in advance, we give it to you within 3 business days after scheduling.
  • You can ask for a Good Faith Estimate before you schedule an item or service. If you do, we give it to you in writing within 3 business days after you ask.
  • If you receive a bill that is at least $400 more than your Good Faith Estimate from the same provider or facility, you can dispute the bill. Keep a copy of your estimate.

How to request an estimate from AriseVida Care

Call 602-899-1229 (relay 711), email info@arisevida.com, or ask our admissions team in person. Please avoid sending sensitive medical details in an initial email. We provide the estimate in writing, on paper or electronically, in the format you prefer.

Clinic hours are Monday–Friday, 6:00 a.m.–noon and Saturday, 6:00–9:00 a.m., Phoenix time. You may also text our nonemergency admissions line at 602-851-6964 at any time; texts are answered as soon as possible and an immediate reply is not guaranteed.

What the estimate is, and what it is not

A Good Faith Estimate shows the expected charges for the items and services you request. It is not a bill, a guarantee of coverage or a contract for care. If the expected items, services or charges change, we give you an updated Good Faith Estimate no later than 1 business day before the scheduled service.

If you have AHCCCS, Medicare, commercial insurance or another form of coverage and use it for your care, this notice does not change your plan's coverage, billing or appeal rules. Our insurance verification team can help you understand your benefits. Applicable billing, coverage, grievance and appeal rights remain available; see our Terms of Service and Client Grievances pages.
