8.1 About this notice
AriseVida Care LLC uses this notice to explain cookies, similar technologies, external website services and the choices available when you visit https://arisevida.com/. Our Website Privacy Policy explains the broader handling of personal information. Our HIPAA and Substance Use Disorder Confidentiality Notice governs protected health information and qualifying substance-use treatment records. Neither this notice nor a cookie setting supplies permission to disclose information that requires a separate authorization or consent.
A cookie is a small item of information stored by a website in your browser. Local storage can keep a setting until it is removed; session storage generally lasts for a browser session. Tags and pixels can send information to a provider without leaving a conventional cookie. External fonts, maps, scripts and other resources can also transmit technical information when they load. A website can therefore share technical information even if your browser blocks some cookies.
8.2 Current tools and available choices
The website's own code sets no cookies, and the site does not run analytics, advertising measurement or call-attribution tools. Its web fonts and animation scripts are served from the site's own address, so opening a page does not send a request to a font or script provider. The only optional external content is the Google Map on the Contact and Location & Hours pages. That map is not embedded when the page opens: the page shows the clinic's written address and a directions link instead, and the map loads only if you allow maps in the on-site privacy preferences (a saved choice keeps applying on later pages and visits until it expires) or press the "Load map once" button next to the address, which loads it for that page view only. Scrolling, continuing to browse or closing the notice is not treated as a choice: closing the notice only hides it for that tab session and grants nothing.
On your first visit the site shows a short privacy notice with three equal options: allow the map, keep the map off, or manage preferences. The "Privacy preferences" entry in every page footer reopens the preferences panel at any time, where you can change or reset your choice. Your choice is stored in this browser only, as one small record described in section 8.7; it contains no visitor identifier and expires 180 days after the choice; when it expires the map stays off and the notice asks again. If your browser does not allow storage, the panel says so and a choice applies to the current page view only. You can also use your browser's cookie and tracking controls and the provider settings described below, and you can contact the clinic by calling (602) 899-1229 to request information or assistance. Keeping the map off, choosing a more private contact method or blocking optional resources does not affect eligibility for care, required communication assistance or grievance rights; every navigation, admissions, insurance, referral and contact function works with the map off.
We do not sell personal information or use patient information to target advertisements. No advertising measurement code is currently deployed on the website. If any is added in the future, this notice will be updated, and its presence must not be interpreted as permission to profile patients or transmit their treatment, insurance, admission or complaint information to advertising services. We review deployed tools against these restrictions and applicable confidentiality requirements.
8.3 Technology categories
Necessary site operation and security. On this website that category consists of the Netlify hosting platform that delivers pages, the form handling that delivers your request to the clinic, abuse protection for the forms (including rate limiting keyed to a hashed form of your IP address; the plain address is not stored), one session-storage item that remembers where you were on the site when you open a notice from the footer, so that the Back link returns you there, a second session-storage item that records only that you closed the privacy notice in this tab so it does not reappear on every page (it saves no choice), and the privacy-preference record that remembers the choice you make in the preferences panel. The preferences panel has no switch for this category: these functions are needed to deliver the pages, handle the forms you submit and remember what you chose. None of these set a cookie. Some technical processing is needed to respond to the request you make. Only functions necessary for that purpose belong in this category. Marketing attribution and audience measurement do not become necessary simply because a plugin loads them alongside a form.
Analytics and performance measurement. Not currently used. The website does not run Google Analytics or any other analytics or performance-measurement tool. If one were added, it could measure page visits, device or browser characteristics, referral sources and website events, and it may use identifiers to distinguish visitors or sessions. We must not treat these identifiers as proof that all related data is anonymous. Website analytics must not receive clinical form contents, insurance identifiers, patient messages or complaint details without the applicable legal permission; a general analytics label supplies none.
Advertising measurement. Not currently used. The website does not run Google advertising tags or any other advertising measurement. If such tags were added, they could associate a website visit or event with advertising activity and may use cookies or similar identifiers. Such measurement is distinct from the company's prohibition on using patient information for targeted advertising. Advertising integrations require appropriate configuration, lawful data handling and any applicable visitor choice. We do not authorize creation of patient advertising audiences, health-based retargeting or uploading patient contact lists to advertising platforms.
Call attribution. Not currently used. The website does not run CallRail or any other call-attribution technology; the telephone numbers shown on the site are the clinic's own numbers and are not replaced by a tracking script. If call attribution were added, it could connect a website visit or referral source with telephone contact and could display a tracking telephone number. Its presence alone does not establish that a particular call is recorded, transcribed or analyzed. Any recording or other additional collection requires its own appropriate notice, permissions and safeguards; this cookie notice is not consent to record a call.
Optional external content (Google Maps). This is the only optional category on the website, and it is off by default. The Contact and Location & Hours pages can show an interactive Google Map; unless you allow it (or an unexpired saved choice allows it, or you press "Load map once" for that page view), the pages show the clinic's address and a link to directions instead, and nothing is sent to Google. The directions link opens Google Maps in a new browser tab on its own; using it is separate from this setting and, like any link to an outside site, sends that request to Google. When the map loads, Google receives technical information about the request, including your IP address and browser details, and may set its own cookies inside the map frame. Turning maps off again in the preferences panel removes any loaded map and stops future loading; it cannot delete information Google has already received. The site's web fonts (Fraunces and Inter) and its animation scripts are copies served from the site's own address and are not external content. External links to social networks, insurers or other resources lead to those providers' services. Their own policies and settings may also apply; our responsibilities for information sent from our site continue to apply where required.
8.4 Health information receives separate protection
Cookie choices are separate from consent to treatment, HIPAA authorizations, Part 2 consents and permission to release records to a referring organization or housing provider. An acceptance button cannot make an otherwise impermissible health disclosure lawful. Common ownership of AriseVida Care LLC and UnityBridge Recovery LLC does not change this rule.
Sensitive interactions include patient identifiers, contact information linked to seeking care, dates of birth, insurance details and card images, service selections, message contents, grievance information and uploaded documents. Website page titles, URLs and event labels can also reveal sensitive context. These data must be evaluated in context rather than automatically characterized as anonymous traffic statistics.
We must prevent impermissible disclosure through both browser and server transfers. Hashing a patient email address, replacing a name with an identifier, or instructing a recipient to discard information after receiving it does not by itself supply the required legal basis. An optional analytics choice does not waive confidentiality rights or authorize session recordings of a patient's interaction with a health form.
8.5 Browser controls and their limits
Most browsers let you view and delete site data, restrict third-party cookies and enable tracking protection. Use the privacy settings for the browser and device you actually use. Clearing stored data can remove a remembered preference; opening another browser or device may require separate settings. Private-browsing mode is not a guarantee that a website, provider or internet service cannot receive technical information.
Because the site sets no cookies of its own, blocking cookies should not affect its features; blocking scripts or browser storage may affect some features, and if your browser does not allow this site to store data, a privacy choice you make applies to the page you are on and the panel says so rather than reporting it as saved. If a form or other function is unavailable, call the clinic for an alternative. Avoid repeatedly sending health information through a malfunctioning form. Blocking browser storage does not necessarily stop scripts, network requests, server logs or information a provider has already received.
A browser's Do Not Track or Global Privacy Control setting may communicate a preference, but it should not be treated as confirmation that every deployed tool has stopped collecting information. Where applicable law requires us to honor an opt-out signal, we must do so for the processing it covers. Such a signal does not submit a medical-record request, revoke a treatment consent or automatically withdraw a signed records authorization.
8.6 Provider information and controls
Google explains its handling of information from websites using its services in How Google uses information from sites or apps. Its cookie information describes cookie purposes and lifetimes. The optional Google Map is the only Google service used on this website, and it loads only after your choice. Google account and advertising settings may affect some Google processing but do not replace the on-site choice.
Because the website does not run Google Analytics, advertising tags or call-attribution scripts, there is no analytics opt-out tool to install for this site. The on-site preferences panel (the "Privacy preferences" entry in the footer) is the control for the optional map. Your browser's own settings remain available in addition: tracking-protection and third-party-cookie settings limit what Google can store inside a loaded map, and clearing site data removes the preference record.
Google publishes its own privacy notices describing the map service and its information practices. Contact AriseVida's Privacy Officer about any third-party website service we use or an applicable request concerning your information. A provider's published policy does not replace AriseVida's own responsibilities or establish that every optional vendor feature is enabled here.
8.7 Provider inventory and retention
The following inventory identifies integrations present on the production website. It is an integration inventory, not a claim that every cookie a provider can use is present in your browser. Storage and collection can differ by page, browser settings, account status, region and the feature used.
Integration | Purpose or function | Information or choice considerations |
|---|---|---|
Google Maps (optional; off by default) | Interactive map of the clinic location on the Contact and Location & Hours pages (www.google.com/maps embed) | Not loaded unless you allow maps in the preferences panel (a saved choice keeps applying until it expires) or press "Load map once" for that page view only; the address and a directions link, which opens Google Maps separately in a new tab, are always shown instead. When loaded, Google receives your IP address and browser details and may set cookies inside the map frame. Turning maps off removes the map and stops future loading. |
Privacy-preference record (avc-privacy-preferences, local storage) | Remembers the choice you made in the privacy notice or preferences panel so it is applied on later pages and visits | One item in your browser's local storage containing only a version number, the on/off choice for each optional category, the date of the choice and an expiry date 180 days later. It contains no identifier and is never sent to a server. Missing, expired, malformed or outdated records are ignored and removed, and an expired record is dropped even on a page that stayed open. If your browser does not allow storage, the panel says so and a choice applies to the current page view only. "Reset my choices" in the panel deletes it. This is unrelated to how long patient, referral or other records are kept. |
Netlify (hosting platform) | Delivers pages and runs the form-handling functions | Receives the technical request information, such as your IP address, needed to serve pages and process form submissions; the site's own code sets no cookies. See the Website Privacy Policy for how form submissions are handled. |
Session storage (avc-policy-return; avc-privacy-notice-dismissed) | Remembers the page and position you came from when you open a notice from the footer, so that the Back link returns you there | One item in your browser's session storage; it contains no identifiers, is not sent to any provider and is cleared when the browser session ends. A second session item, avc-privacy-notice-dismissed (value 1), is written only when you close the privacy notice with its close button; it records no choice, grants no permission and only keeps the notice from reappearing on every page for the rest of that browser session. Both are cleared when the browser session ends. |
Page and form components | Deliver public pages and requested features | Necessary processing only; these components do not set cookies and no optional measurement is bundled with them. |
Cookie lifetimes and the retention of information already sent to a provider are different. Session storage may end when the session ends; persistent storage can remain until expiration, removal or browser restrictions. Clearing cookies does not necessarily erase a provider's server records or legally retained healthcare records.
Provider defaults are not a statement of our actual configured retention periods. The website's own session-storage items last only for the browser session; the local-storage preference record expires 180 days after the choice. Any cookies or storage set by a third-party provider when its resources load are governed by that provider's documentation. Google describes different lifetimes for different technologies. Ask our Privacy Officer for information about a particular tool, data category or applicable deletion request. We must align actual retention with the purpose, legal requirements and our retention procedures.
8.8 Questions, requests and accessible assistance

Clinic telephone hours are Monday–Friday, 6:00 a.m.–12:00 p.m., and Saturday, 6:00–9:00 a.m.; Sunday closed, in Phoenix, Arizona time. Patients may use the 24/7 text line at (602) 851-6964 outside clinic hours for nonemergency contact. This does not guarantee an immediate clinical response. Do not include medical records in a cookie-support message. Ask staff to arrange an appropriate method for sensitive supporting material.
8.9 Changes to website settings
We update this notice when the deployed tools, their purposes or available controls materially change. The on-site preferences panel describes only tools that are actually deployed, and it reports a choice as saved only when the record was stored; if storage is unavailable it says that the choice applies to the current page only. If a new optional tool were ever added, it would be listed here and in the panel, would stay off until chosen, and its version change would ask returning visitors to choose again. Any legally required consent or renewed choice must be obtained through the applicable process.
The preferences panel changes only what this website loads. It does not change a browser setting, and withdrawing a choice does not retrieve information a provider already received. A privacy request applies according to its scope and applicable law; it does not retroactively undo information already lawfully processed.
