INFORMATION & YOUR RIGHTS

Website Terms of Service

13 sections

Effective date: September 8, 2026

2.1 Purpose, operator and scope

These Website Terms of Service describe the rules for using the AriseVida Care LLC website and its public information, downloadable resources and online inquiry features. "AriseVida," "we," "us" and "our" mean AriseVida Care LLC. The site covered by these Terms is https://arisevida.com/ and any AriseVida-operated page that expressly links to these Terms.

Please read these Terms before using a website feature. Some features may present additional terms and request your affirmative acceptance before you use them. Privacy notices describe our information practices; receiving them is not, by itself, a consent to treatment or release of health information.

These Terms govern website use. Your actual treatment, informed consent, financial obligations, medication management, telehealth participation, program participation and any housing arrangement are governed by the separate documents and applicable laws relevant to those services. If there is a conflict, applicable law and the relevant clinical, privacy or housing rights control. These Terms do not waive a patient's rights or replace a required disclosure, informed consent, HIPAA authorization or Part 2 consent.

2.2 Emergencies and urgent needs

This website is not an emergency service. Do not use a contact form, admissions form, insurance form, email, website feedback channel or grievance submission to obtain an immediate medical response. If you have a medical emergency or there is immediate danger, call 911 or go to the nearest emergency department. In the United States, call or text 988 for suicide or mental-health crisis support; use 911 for immediate physical danger or a medical emergency.

Website messages and requests are not continuously monitored for clinical emergencies. A reference to an admissions helpline or after-hours contact does not mean that an online form is monitored continuously, that a clinician will respond immediately, or that emergency services have been dispatched. If you cannot reach our team and the issue is urgent, use an appropriate emergency or crisis service.

2.3 Educational information and clinical decisions

The website contains general information about behavioral health, addiction, medications, treatment options and recovery resources. It cannot account for your full history, examination, medications, risks or personal circumstances. It is not individualized diagnosis, treatment, prescribing, medical clearance or a substitute for evaluation by a qualified professional.

Do not start, stop or change a prescribed medication based solely on information on this site. Information about methadone, buprenorphine/Suboxone, naltrexone, psychiatric medication or Deep TMS describes possible services and considerations; it does not establish eligibility, clinical suitability, a dose, a prescription, or an obligation to provide a particular intervention. Decisions about controlled substances and opioid treatment are subject to clinical judgment and applicable requirements.

Our references to peer support, case management, supportive housing, justice reentry or community resources do not create legal representation, probation supervision, emergency shelter placement or a guarantee of services from another organization. Resource content may become outdated. Ask the relevant provider or agency to confirm availability and current eligibility requirements.

2.4 Admissions, appointments and referrals

Submitting "Start Admissions," "Verify Insurance," a referral, a request for a call or a contact message is a request for follow-up. It does not complete admission, establish that you have been accepted into a program, reserve a bed, create a treatment plan, or confirm an appointment. A website acknowledgment means only what it expressly states; a receipt or confirmation screen must not be relied on as evidence of clinical review.

Our team must confirm the next step. Admission can depend on assessment, medical necessity, program scope, age and other lawful eligibility criteria, availability, staffing, required documentation, payer requirements and informed consent. We apply criteria consistently with nondiscrimination and reasonable-modification requirements. We do not promise a particular start date, location, treatment outcome or service duration through general website content.

If you make a referral for someone else, identify your role and ensure that any shared information and requested disclosure are authorized under applicable law. We may need to verify identity and authority before discussing the person's care or confirming whether that person is receiving services. Providing transportation, funding or a referral does not itself grant access to clinical records.

2.5 Insurance, charges and financial information

Insurer names and logos describe general relationships or options and may not apply to every product, network, location, clinician or service. Coverage depends on the member's specific plan, benefits, eligibility on the date of service, authorization requirements and other plan rules. An eligibility response or preauthorization does not necessarily guarantee final payment.

We explain applicable financial information through our admissions and billing process. Do not assume that a website estimate, a "most insurance accepted" statement or a verification request means all costs are covered. Any actual patient financial responsibility is governed by the applicable agreement, payer contract and law; these Terms do not authorize charges prohibited by Medicaid or another program.

Patients who qualify for a good faith estimate under applicable law may request and receive the required estimate and dispute information; see Your Right to a Good Faith Estimate. Any statutory billing, coverage, grievance or appeal rights remain available. Website use alone does not establish a payment obligation, authorize a payment card charge or assign insurance benefits.

2.6 Supportive housing and related assistance

Supportive housing is operated by UnityBridge Recovery LLC, a separate legal entity under common ownership with AriseVida Care LLC. Housing services are governed by the applicable housing operator's agreements and responsibilities. Common ownership does not itself authorize sharing treatment records or make the entities interchangeable.

Housing, transportation and community-support inquiries do not create a tenancy, bed reservation or transportation entitlement. Actual arrangements are governed by their written terms and applicable law, including nondiscrimination, accommodation, discharge and housing rights. You may use our grievance and safety-reporting channels to raise a concern without retaliation.

2.7 Information you submit and safe use

Provide accurate information to the best of your knowledge and keep your contact details current when seeking follow-up. Do not impersonate another person, submit fabricated referrals, upload harmful software, or knowingly send information you have no right to disclose. If you make an error, contact us so we can help correct it through the appropriate process.

Before providing records or insurance-card images, ask our team which upload or other transmission method is appropriate. A visible upload button or ordinary email address is not a representation that the entire delivery route has been independently verified as secure. Do not submit passwords, payment-card security codes, full Social Security numbers or unnecessary clinical details through a general message field. If a form fails or a submission is uncertain, call the clinic to arrange an appropriate alternative instead of repeatedly submitting sensitive material.

You retain your rights in information and materials you lawfully provide. We process submitted materials only for the requested service, lawful administration and purposes described in our privacy notices, and only when the required legal permission, authorization or consent is present. These Terms themselves do not supply a HIPAA authorization or Part 2 consent. Submitting a patient story, image, testimonial or review privately does not give us unrestricted advertising or publication rights. Identifiable patient publicity requires any separate written permission and health-record authorization required by law.

2.8 Appropriate website use

You may use the site for lawful personal, informational, referral or professional purposes consistent with these Terms. You may not intentionally interfere with the site's operation, introduce malicious code, bypass access controls, attempt unauthorized access to records, impersonate staff, harvest personal information, or use our forms for harassment or unsolicited mass messages.

You may report suspected vulnerabilities through the clinic contact so we can arrange appropriate handling. A public website or this reporting invitation does not authorize intrusive security testing, access to another person's records, downloading confidential data, or disrupting systems. A good-faith complaint or honest criticism is not misuse merely because it is unfavorable to AriseVida.

2.9 Intellectual property and downloadable resources

Unless otherwise identified, the site's text, original graphics, design and branding are owned by AriseVida or used with permission. You may view the site and download resources made available for your personal, noncommercial information or care-related use. Keep attribution, copyright notices and limitations that accompany a resource. This permission does not transfer ownership or grant a right to use our name or logo to suggest endorsement.

Third-party names, marks and materials belong to their respective owners. Public-domain materials and uses permitted by law remain subject to those rights. Contact us if you believe content on our site infringes a right you hold, identifying the material, its location and the basis of your concern. We review sufficiently specific reports and take appropriate action.

2.10 External services and communications

Links to an insurer, mapping service, social-media page, referral resource, payment system or patient portal may lead to separately operated services with their own terms. A link is not a guarantee of that service's availability, security, clinical quality or accuracy. Our obligations for vendors handling information on our behalf remain governed by applicable law and our agreements; describing a service as third party does not remove those obligations.

If a patient portal or telehealth service has separate terms, those terms apply to that service to the extent lawful. These public-site Terms do not provide informed consent to telehealth, authorize audio or video recording, or establish that a clinician is licensed to treat a person in every state from which the site can be accessed.

If you request contact, we may respond through the method and scope you select, subject to applicable consent and confidential-communication requirements. Consent to optional marketing must be distinct from a request for care. Message or data charges may apply under your own carrier agreement. Do not assume that replying to a general text number reaches a clinician or emergency service.

2.11 Availability, corrections and website limitations

We work to keep the website useful and accurate, but content may contain errors or become outdated, and technical features can be interrupted. We may correct content, change available features, perform maintenance or suspend a feature that creates security or operational concerns. We provide reasonable alternative ways to obtain information or request assistance through the clinic.

To the extent permitted by law, general website materials are provided on an "as available" basis without a promise that every page will always be complete, current or uninterrupted. We do not guarantee a result from treatment, an insurer's decision, a bed opening or a third party's service. This paragraph does not disclaim the professional standard of care, contractual duties for actual services, required accessibility, record confidentiality, or responsibilities that cannot lawfully be disclaimed.

To the extent permitted by applicable law, AriseVida is not responsible for losses caused solely by your misuse of the public site, inaccuracies in information you knowingly supply, or independent third-party outages beyond our reasonable control. Any limitation is applied only to the extent lawful and factually justified. Nothing here excludes liability that cannot be excluded, excuses our negligence or misconduct where a waiver is prohibited, or limits remedies for privacy, civil-rights, billing or other legally protected claims.

2.12 Concerns, governing law and preservation of rights

You may contact us to resolve a website concern and may use the Client Grievances, HIPAA, Nondiscrimination, Accessibility, and Safety or Quality-of-Care notices for their respective issues. You are not required to complete our internal process before contacting a regulator, an attorney, law enforcement, an advocate or another legally available external forum. No part of these Terms imposes a mandatory arbitration agreement, class-action waiver, release of clinical claims or shortened statutory filing period.

Arizona law and applicable federal law govern these Terms to the extent that choice is permitted. This provision does not deprive you of a nonwaivable protection under another applicable law or require a complaint to be filed in a forum different from the one established by that law. If a provision is unenforceable, the remaining lawful provisions continue to apply to the extent they can operate consistently with the purpose of these Terms. A failure to enforce one provision is not a permanent waiver of it.

We may revise these Terms and display a new effective date. Material changes requiring renewed acceptance will be presented for that acceptance. An update does not retroactively waive an accrued claim or amend an existing clinical, housing or financial agreement without the required process.
